Some Fixes - Multiple File Upload using PHP

Hi Team,
We are trying to implement multiple file upload using following:
[url removed, login to view]
We would like to have following feature on this:
1. for each upload, file will be saved into new folder
2. Folder name will be come from the database
3. File will be saved as foldername_1, foldername_2 and so on
4. after each successful upload, application will refresh the screen
5. earlier uploaded image will not be visible to the user
