Need a survey script that does the following:
1. ADD new client; business user
2. Create/ add/ edit / delete surveys - can make surveys that use text boxes , option buttons, check boxes, drop downs, text fields etc.
3. Each (cleint)user gets the reports emailed to them automatically at end of month or admin can email reports
User interface: There needs to be (2) different interfaces
1. Clients will be able to administer these surveys at there place of business,so a person can just automatically begin taking the survey.
2. Users can register for surveys online and then serach for the right survey by state and then by business name.
** Users will not see survey results**
1. % of each question answered for that users
2. % of each question answered per state compared to user
3. % of each question answered across the US compared to user
Please feel free to share any examples of past work. Looking for a solution that can be customized to match current website. User interfaces should be customizable based on business (client) if possible.
** REVISIONS -- REPORTS
1. % of each question answered for that client

3. Show overall resutls for each quarter based on each individual client, and every client together
