small Python PDF generator using PyPDF2

This is a very simple project that requires a python script that will use PyPDF2 in order to generate a single page form as in the attached document. Essentially, I only need 18 string variables placed as in the attachment, and only one graphic file, again as placed in the attachment. You will create a function createcustomform() that will be generating a PDF document in a given location.

I meant to say that you can use PyPDF2 or ANY other open source library that might be easier to perform the task! But NO commercial libraries please.
