2 Opt in boxes (lead generators) for website. 1 Professional looking newsletter sign up box and 1 Contact sign up form for 1 Week of free training - repost

I'm after 2 very professional looking opt in boxes for my WordPress website. newsletter sign up form: (colors: white, blue, yellow, orange, just like the homepage) I'm after a call to cation box to download a Free Nutritional guide. My website is [login to view URL] The opt in box should be placed in the top right corner of my website. Top right next to the picture of myself. It should say: ''After a great body and bursts of energy?'' No gimmick, No quick fix, No magic bullet diet scheme. (might change) underneath that I'd like to have: ''PICTURE OF MY NUTRITIONAL GUIDE displayed on an ipad'' (eg.: [login to view URL]) You should be able to design a small cover photo: caption: Nutritional Guide - ''You are what you'' eat. picture of myself which I'll send to you with some fruits and vegetables and maybe a scale with measuring tape eg. the picture displayed on my homepage can be used for it. Underneath the picture: ''box for email address'' saying: ''enter valid email'' and DOWNLOAD FREE BUTTON (eg: [login to view URL]) with secure symbol - your email will never be sold, rented or spammed My autoresponder is with Mail chimp. I would like to use the opt in boxes on other pages so it should be possible to integrate them easily on other pages as well. I'd also like to have a round looking FREE TRIAL WEEK button that I can place anywhere on my website and will link to the sign up form. Ideally next to the button in white handwriting style it should say: Give It A Try! (see [login to view URL] top right corner) The button should be placed in the top left corner of my front page.
