497501 Website Design Dental Office

We need a person/company to develop a webpage for an upscale dental office. We already have the code logo and and backdrop/style but we are open to other options as well. We would need an image gallery (pics provided by us), a bio page, contact page, site map, location map w/office hours services page and maybe 1 or 2 misc others. Please provide samples of previous work as well as a time frame to have job finished. Please also let us know your pricing and credentials for maintenance and hosting. We have no use for email accounts so that will not be necessary. Include your fee for revisions as well. Thank You
