Basic website with online petition

This is pretty straight forward: I need a good looking website designed with a BLUE background and custom graphic at the top. I will provide 2 photos (attached). I want the hat in th e photo of the person to NOT say LSU, but to Photoshop the " M " from the other photo into the spot where it says LSU on the hat. Then the whole title banner needs to have that photo on the left and the title "Les Go Blue" to the right. Below that I want 3 spots for content. I need a box that spans the whole page for text right below the banner. Below that, I need a box that takes up 1/2 the for PayPal donations. To the right of that, I need a box that links to an online petition hosted on the site. Yes, I need a functional petition created that will be hosted on the site. So in short, I need a banner made (easy). 3 boxes for content (easy), and an online petition created (hardest part of project). Thanks.
