Develop user interface for an enterprise application -- 2

We are looking for a UI/UX developer who is proficient in developing easy to use user interface for an enterprise application. The application will be "form" intensive and should be appealing to international audiences.
The user interface should be developed using HTML5/CSS, Material Bootstrap, and Angular framework. The design should provide at least 2 themes - light theme and dark theme. The design should allow the addition of more themes in the future.
No proprietory or licensed components should be used in the development phase.
As part of the deliverables, the developer will provide the following artifacts
1. All the source files and images used for development.
2. Standard set of icons
3. All form elements - eg: textbox, checkbox, text area, radio buttons, drop-down list, select2 drop-down list, multi-column drop-down, grouped drop-down, multi-selection, dependent list, toggle button, buttons, text box with chips(tags), file upload and other relevant form elements.
4. All UI elements like - tabbed panels, show-hide sections (accordion), wizard workflow, charts, Grid, Tables
5. Dynamically/programmatically managed multi-level menu used for access restrictions.
6. Error messages, warning messages, alert box, confirmation box, field validations.
7. User profile view, login, and password reset.
Screen details are provided in the below-given screen mockups (refer to URL). Screen components not covered in the mockups should be created on a separate screen.
