Small Project - Need already made header placed on INvision Header

I have this page set for new design > [url removed, login to view] for new site design.
I for the life of me cannot get what I require below (But normally do all my own stuff)
1. I would like the header table in link shown above placed for Header to my Invision forum here > [url removed, login to view]
I cannot get it right I consider this a small job, & would just like this done.
To the winning bidder I have lots more of custom html & some php database work.
I have been in past let down or delayed & would like this done asap.
