Weebly Website Form Interface with slider (example given)

I'm looking for a Form Interface for my website which is build using [login to view URL], my website is [login to view URL] and I am already have a basic form as you will see on the "Get Advice" page - [login to view URL]

I am looking for something more like the form interface on this page - [login to view URL]

I need the form entries to be sent to my email address (weebly's forms do this already, and they also can be seen in the form section in my weebly account).
