The application should generate a report containing the conclusions of all answered questions when a request is complete (all necessary answers filled in)
Unfinished request should be visible in a survey so other employee can open them and finish the unanswered questions.
Answers to your questions:
1) All answers for a single request (23 questions with subquestions) should be stored in a single record. So every record in the re_request table contains one request.
2) All form objects already have a Control Source, so you can see which field contains which data.
The table re_request contains all requests, for question 12 there is a separate table which contains all companies that are not blank, the relationship is foreign key co_re_id to primary key re_id
3) The report contains a list of conclusions, everywhere in the scanned pages where it says "conclusie" (dutch for conclusion) you can write down the selected answer or the written answer in case of an open field. example: question 3 = N and "niet bedrijfsmatig", then the conclusion contains "niet bedrijfsmatig". Or question 10 = N, then the "omdat" (Dutch for because) field can be used as a conclusion.
