I need an employee badge template that shows the following:

Front side:
Cloudwi Logo
Head Shot (Photo)
Full Name: eg. James Smith
Job Title: eg. Field Technician
Employee ID#: eg. 234

Backside:
Cloudwi Logo
Mailing Address: P.O. Box: 555, Ottawa, ON N1N N1N
The bearer of this identification badge is a Cloudwi employee. This ID remains property of the company and must be surrendered upon separation or termination of employment.
