117822 ASP On-line registration form

This project is TIME SENSITIVE and needs to be done within 24 hours. Please do not bid unless you are prepared to start immediately after being chosen. I will not escrow, so please don't ask. You will be paid as soon as I thoroughly test to make sure it works as I described.

I need a registration form created that mimics the attached company registration form. This is very similar to another conference registration form used last year and located here [url removed, login to view] except this one will ask for a lot less information.

Confirmation of registration will be sent to the applicant

A copy of the form gets sent to the admin

You will need to place the completed form and database on the development server. And give me info on what needs to be done to place it on a Production server.

I will hire you again to complete the next phase which is to create a second form and an automatic emailed link to it after a user registers. So when an applicant registers the individual delegates on the company registration form, the individual delegates will be emailed the link to the second form which will also be input into the database, so please code with that in mind if necessary.

I can give you FTP access to the former database and forms

You must comment your code so that I can add fields to the form later if needed without problems (I know very little programming, but enough to understand some of it if I look hard enough).
