50258 Design an IDE for php / html

I am looking for someone to design a GUI for form design for php / html The form generator should be a stand alone application that installs. The application should allow for a set of questions to be asked (wizard) determining what questions are to be asked on the forms, then generate the appropriate form. Should support the following OPTIONS: 1) multi-page forms (filling out a form on one page takes you to another set of question on the next page for another form with ability to record to separated tables, and allows for identification using either a USER ID or USER NAME. 2) support mailing the form results using a clients email application or with php the build in mail functions, and / or should also support the ability to submit information to a database. Should support both abilities. 3) Database formats should include ability of: the following database (MySQL, MSSQL, PostgreSQL, MSQL, DBM) 4) Support the option of validating the results. (for example the (at) symbol for email along with a . in the domain side) or (phone number being all numbers) or (not null) or (number of characters if applicatable) if the form is html based, it should provide the appropriate javascript for verifying the form results, if php can use either javascript or php verification.functions. 5) Should generate the entire form as a single page (php). Should also have the ability to generate a “submitted page” or enter a web page to go to after submitting the form. 6) For multi-page forms the form should be viewable in the IDE in the same format as what powerpoint does, all the forms on the left side, when a form on the left is selected, the code is loaded on the right side (bottom), with the browser output on the top right side. This should have the ability to parse the php code, to display the form in design correctly. 7) The wizard should also set the whole page to a CSS that can also be edited. In essence the resulting application should be able to generate either a single page form or multipage form in a easy format. The whole application should be done in a simple wizard format, then have the ability to edit further the files by hand. It should also generate the sql table(s) as a sql script also should allow the user the ability to connect to the db server, and create all tables and fields at the same time. It should also automatically generate a basic retrieval script to read the form information in the database to the screen, it should allow the ability for edit, delete the information.
