Manual for customer service course

I have an outline for a manual for a course in customer service. I need someone to develop the manual based on this outline. I will then add exercises, etc, for use in a training situation.
See outline attached. Word count for total project is expected to be 5000 to 8000 words.
The manual needs to be easy to read and to-the-point e.g., use of sub-headings, dot points, lots of white space, etc. The language needs to be straight forward English – no flowery language please – with a conversational tone. The audience will be adults of various backgrounds and literacy levels with some experience in the workplace.
Your English will need to be excellent (using UK English). This is a work-for-hire arrangement so authorship and copyright revert to me. Original work only – no copy and paste even from public domain works – although I do not mind you using various sources as long as they are properly reference and / or rephrased.
Deliverables preferred in MS Word format using the same template as the attached outline (although this is negotiable – let me know your thoughts).
Hoping for $150 maximum. Please provide samples of your work.
