Form creation for Mobile Phone to Desktop spreadsheet for Material picking list

We are a scaffolding company that require a simple user friendly Mobile phone (ios and android) form that a site/field worker can input quantities into a form on their handheld device, click send and automatically have that material list sent to an email address for the form to be downloaded onto a desktop PC in our Office in the spreadsheet format that we currently have set up.

The mobile form would also require the ability to input a Job Name, Site Address, Contact Name, Delivery Date. It would also be good if the mobile form showed the total weights of each item, next to the quantity (item weight multiplied by quantity).

The spreadsheet we use is attached and a sketch of the mobile phone form.
