Convert iPhone app to HTML5 and Android mobile apps

We have an iPhone app near completion that has two main pages (Check-In, My Profile) and two secondary pages (Find a Gym, Settings). The app allows users to check in via GPS at gyms, see their workouts on their profile, and find local gyms on a map, plus change the number of days they want to work out.
We would like to convert this iPhone app to an HTML5 mobile app, which can be used on any smartphone. In addition, we would like an Android native app as well. Please give quotes for this conversion, and any links to previous mobile work done. Thanks!
