222556 Form Issues

Hi

There is an online 18 page form, which generates a pre-populated pdf after filling up by the user. After submitting the form the pdf has been generated which has been directly sends to the administrator email id with user details. As if now the pdf has some issues that needs to be resolved. It is not able to call values from the database. Also some spacing and alignment issues. The form and pdf are already done only requires some finishing .Please quote accordingly.
