Create a Wordpress Slider With Multistep Form -- 2

I'm looking for a wordpress plugin to convert the main basic image slider with a custom slider with image on left or right and a contact form on the other.
The slider should have the option to turn on or off the Image and leave only the form on the entire space if needed.
Should also have the option to turn on or off the icons on bottom of the slider and ability to edit each icon/text next to the icon and explanation popups for each icon. Popups should have Text and Image placement options just like it is in the Example url.
Form should have the following functionalities:
Text, Text Boxes, Email, US Phone (format: 0000000000) Address, Zip Code (Format: 00000) The form should pick up the City and State based on Zip code entered and show the pulled city and state with EDIT text link/button so if they need to change it they can. I will provide the list of all US zip codes with city and states in MS excel format to be integrated.
The form should also have all the functionality such as Date picker, IP, Time Stamp and URL capture.
The form should be able to store collected information in a database within WordPress for easy export and viewing and should also be able to post the data to V-Tiger CRM.
