Create action after video ends and change information based on form

There are 2 parts to this job:

1) Program attached web page ([url removed, login to view]) that when the video ends an ‘information box’ appears over it or in its place - whatever you think is best and most doable.

Details:

- The button part should be click-able to a link

- Here are 2 examples of pages that do a similar thing to what I want:

2) Create a form that will determine what will be on the information box when the video ends.

Details:

The form will do 3 things

a) Create a new document based on the attached page template ([url removed, login to view])

b) On the new page/url created - the data entered into the form will change a few variables

c) An email will be sent out with the newly created link - a few variables in that email will change based on the data entered into the form.

Information on the form:

1) Center Name: [will replace x6 on the [url removed, login to view] page]

2) Name of your City:

The information from the above 2 fields will create the new URL: [url removed, login to view] --- Also - x10 will be replaced by the new URL

2) Phone Number: [will replace x9]

3) Website: [will replace x11] (should already have http://)

4) Email: An email will be sent out to this address after form is submitted with new URL for page. Please create a template document that I can insert the text for the email that goes out. In that document the x6 x9 x10 and x11 variable should work. For example if I write in the email: Thank you x6 for using our services. It would say “Thank you Center Name for using our services.”

5) Link you want viewers to click on when video ends:

6) Words you want written on ‘ending button’ that leads to your link: (limit to 50 characters)

7) Information you want to appear on screen at end of video:

(limit to 23 lines with a maximum of 35 characters per line)

NOTE:

1) If a new form is created with the same ‘Center Name’ & ‘City’ it should create a new file replacing the old file with the same name/city.

2) z1 - that will be replaced by me on the template page that creates every other page - don’t worry about it

3) Feel free to change the x6 x9 x10 x11 with some other TEMP variable if it makes things easier.
