I need a form for customers to complete to fill in a family tree design. I need the informaton, once completed and submitted to be placed in a graphic that is designed and is in an eps? format with designated areas for copy and photos.

I need a programmer that can give me consultation as to the best way to achieve the end result and then do the programming.
