121536 simple fix

Hello,

I need someone to update my site as follows:

1. I have a form that visitors can fill out and I need to simply reduce the number of required fields from approx 20 to about 4-5.

2. I want this form accessible from the home page instead of going to my sellers page to fill out the form. Basically I want a button they can click on from home page to take them directly to the form.

That's it.

I will pay in full upfront via escrow for those with good ratings which I will give preference for this project.
