I need one page web based form created. There are just 9 fields to be be filled in. I need this information stored in a MYSQL database. This must be developed in PHP/HTML/CSS.
THIS MUST LOOK EXACTLY AS PER THE ATTACHED PICTURE.
IT MUST BE RESPONSIVE.
SEE THE ATTACHED WORD DOCUMENT AND PICTURE FOR ALL THE DETAIL.
This is a small project so I need it done as fast as possible.
I will need you to work on my TEST SERVER. I will send you details.
Once you send me the a link to test it I will test and send feedback for you to tweak/resolve.
I will keep testing till its 100% to what I want.
