Hello, looking for a serious programmer to make a standalone guestbook with the form and functionality of this calendar example on my site,

I will need the following:
Data stored in a flat file, no database.
Separate header and footer so I can customize the guestbook to match an existing site.
Required fields include:
· Name: (First and Surname)
· Email Address:
· City
· State
· Country
· Comment
Once comment is left an Email notice sent to guestbook owner so they can approve/delete/modify the comments left before the comment is listed on page.
Default date for comment will be determined once “submit” button is clicked.
Must have an admin area so guestbook owner can edit, or delete comments. Fields I would like to be able to edit would be same as required info listed above. Admin area should also have an area for guestbook owner to set their email address notice of new comment and number of comments left per page.
Working demo on your site will be required before final payment is made.
You will deliver source code and I will own program after completion. If you have any questions, please use project message board. You can use templates from web address above.
