Massage Tracker - Using PHP and jQuery

For this project you must use php for the server side code and jQuery + javascript for the client side. No Bootstrap-like frameworks where presentation layer is put into HTML. Clean separation of HTML and CSS. It is okay to use jQuery plugins that are freely distributed. Application should be responsive in order to support computers and tablets.
Basic mySQL tables include: 1) user table that keeps track of who has access to the system along with option for password recovery via email sent with link to reset password. 2) login_attempt table to track each time someone attempted (successfull or failure) to login. 3) client table that keeps track of the main client information.
To track the massages, it will require three tables, which I already have defined: 1) massage_visit, 2) massage_service, 3) massage_payment. There will be one row for each visit made by a client in the massage_visit table. Then for each of these visits there will be 1 or more rows in the massage_service table. And for each service there will be 1 or more rows in the massage_payment table.
The attached MySQL file contains the create statement for each of the tables along with create statements for log tables (one for each table that will monitor changes to the other tables via triggers - which is also found in the MySQL code).
