Save email address from online form and other data to a text file

Hello,
My project relates to customizing an online form.
The online form currently requests an email address. When a user has typed their email address into the form and clicks "submit," the user's email address is sent to me in an email. I would like the following information added into the email that the form sends me:
1. The domain name the form is located on (as I use the same form on multiple domain names)
2. The time and date the form is submitted.
3. The user's IP Address.
The email the form currently sends me looks like this:
"Form details below.
Name:
Email Address: [url removed, login to view]
Subject:
Message:"
I would like it modified to this:
"Form details below.
Email Address: [url removed, login to view]
Domain Name: [url removed, login to view]
Time and Date: 23:45, 19/07/2014
IP Address: [url removed, login to view]"
In addition to the above, I would like every form submission saved to a comma separated text file on the server, formatted like this:
email address, domain name, time, date, IP address
For example:
[url removed, login to view], [url removed, login to view], 23:45, 19/07/2014, [url removed, login to view]
[url removed, login to view], [url removed, login to view], 23:46, 19/07/2014, [url removed, login to view]
[url removed, login to view], [url removed, login to view], 23:47, 19/07/2014, [url removed, login to view]
