Wordpress - require email and name to download files

Client is using WordPress 4.5.2
They have 4 files (PDFs) that they want to offer visitors to download, but they need Name/Email first.
I'm looking for a really simple solution. Simple form, links sent to registrant via email, and email sent to client with registrant Name/Email address. I don't need a custom plugin. Use whatever free ones are available to keep the price down.
I have Download Monitor installed. It keeps count of the downloads. Would it be possible to use the DM links for the files?
