Create form in XHTML with JavaScript validation

See the attached document. I need someone to build this form for me in XHTML. The following are requirements:
1. Code should be clean, plain XHTML. Table layout is ok as long as it's clean.
2. You do not need to create any design or CSS styles, however, similar looking elements should be given a class so they can be styled easily later (e.g. all text input boxes should have the same class).
3. Headings for each section should be <h4> tags.
4. All form fields need to have the name property set correctly. E.g. question 1 under the "Forehead" section could be named "forehead1".
5. Most of the elements will just need to be radio buttons, although you may use dropdown boxes for any of the questions with more than 3 responses if you like.
6. There should be a submit button at the bottom. The form should have simple JavaScript validation triggered onsubmit to make sure that every field has been filled out. The final question for "men only" should only be validated if the user has said that they are male. If the user has not filled out everything, they should get a popup telling them which fields to fill out.
I need the form built within 24 hours of project starting. Payment will be made by escrow and released when I have checked that the form meets all requirements. Please do not send me a template bid to apply to this project, and please show me at least one or two examples of forms you've made. If you know PHP, I may be able to give you some additional work on this once the HTML form in build.
