Tablet Application for Internal Use - iOS or Android

I have a small construction business and I take a lot of phone orders. I want to have an order taking app made for either the iPad or an Android tablet for my use only. This app should work similar to a basic web form. In the first version, I just need an app based form that records the options selected and combines those selections into a plain text document that is emailed to an editable address. In later versions the app will need to be able to calculate pricing based on selections/entries. One of the variables that can affect pricing will be distance from an address. I imagine this means having to tie into a mapping API. Long term, I want to build the app so that it can also handle invoicing, route optimization, and scheduling. I know that you will need a lot more information to begin work but please let me know if you need more information to decide if you can help with building this application.
