Need a GO DADDY Database Built

Hello: I use GO DADDY Windows for hosting and need a database built for clients to view reports online that I input after completing. I will need to give each client their own user name and password to login to my website to view all of the completed reports. The report is a one page paper form that I fill in manually..Obviously the database will need to store the completed reports on my server for the client to view when they need to do so. I previously awarded this project to someone who couldn't get it working so if you do not know what you are doing, please do not bid..The website that it will be needed on is [login to view URL] and it would need to be completed as fast as possible..I have attached the one page report that I fill in that will need to be entered in the database for clients to view.
