Filemaker or mobile app creation

Currently, we have security guards go to [login to view URL] and enter the information to an online form. Tis form is in Formsite. We import the data from the formsite results to Filemaker and we print out weekly reports. We want to make changes to the filemaker. 1. If possible, replace the formsite form with a Filemaker web form that captures the data directly. 2. Currently, we only track three things, 602's, Warnings and 602A's. We need to adjust the db (sample provided) to reflect the additional categories and print out reports. The ultimate goal is to take all this data and create online access so our guards can not only enter new data, but look up information on their mobile devices. This will be an ongoing project that will make gradual changes to this file over time. We want to adjust the file and then beta test it for a bit, then adjust and move on. We also need to create a way for anyone who is going to enter information into the system has their own unique ID and password. When they register to use the system we will want to capture all their contact info and keep it on file. I had this project posted and it was taken down because they said the description wasnt good enough. I need someone who can invest 5-10 hours a week and work with us to improve this program to meet our needs. As amateurs, we have setup a db that works but we have reached the zenith of our ability and need a db tender to take over. It's a very small, simple db, but we have several projects. We want to add some report features and fields to the db and if possible, have the database setup so information can be added directly via a web form without the need to import. Would also like to schedule weekly data output and distribution via email and be able to easily control who receives the emails. This will be an ongoing project as I have several small projects like this. As quickly as we can, we would like to make this into a mobile app. We have No problem using oversees providers but please, be available for questions and such without a long 6-12 hour wait. As this will be an ongoing project, please bid at 10 hours for the first week. If you would like a copy of the blank db, one can be provided. Any referrals to previous work would be appreciated.
