127570 PHP/mySQL App from Scratch

Initial Request for Bid. I need a PHP mySQL application coded from scratch. I have a fundemental design document and a spreadsheet listing database tables and fields. Please note I have some experience developing with PHP mySQL and is part of my own job responsibilities. Please consider this when bidding. My initial design is for: 1. 8 html php application pages that interact with the database. 2. The database will have 5 tables that I am aware of. Here are some basic requirements for the application: 1. The application must be delivired in a form where it can be installed from scratch on any server. 2. A config file must be available for database table creation and database access. 3. A script to create the tables must be provided. 5. A secure login page to the application must be provided. 6. Website pages within the application must be secured. 7. The website pages must interact with the database as specified in the design document. 8. CSS must be provided. The CSS document will provide for large text fields with background colors, larget text AND where the applications shows a list, I would like every other row to have a rotating color scheme. 9. Upon completion, I should be able copy the delivered files to a new server, create the database manually (using cpanel), modify the config file, run the table installation and successfully access and use the applicaton. 10. The developer should be able to change and improve the design as needed. 12. If canned modules or functions are used, they must be copyright free. ADDITIONALLY: The developer must provide an explanation of all files used in the application. I need to know what each file is doing. 11. Most importantly the developer should be open to providing daily updates and welcome messenger communication via Yahoo. I will provide a test server OR the developer can provide one. Before bid is completed. I will resubmit the requirments (which may be updated at that time) for the devloper to sign of and agree to.
