Code Distribution and Verification

Develop a data collection website and linked access database. The website must follow our corporate design and layout requirements (all graphics and template will be provided), also create an access database to collect and manage the data. The website will contain a form that will allow the user to input 4 or 5 fields of data, including an email address. The system will check the email address to ensure it is unique, then send an email to the user with a link to a secondary site to verify their address. Once verified, the system will pull a unique access code from an existing table, associate it with that email address, and send an email to the user with that unique code and an HTML link to another website. We will provide all layout guidelines and graphics for our corporate look & feel, as well as all copy for the websites and the emails. Additionally, we will provide the list of unique codes that will be delivered. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Website design to be done in Microsoft Frontpage with the database being done in Microsoft Access
