Repair: Customize Your Order Cart

We have a cart needing repaired on our website that allows the user to customize their order. They are then taken to PayPal to complete the transaction. We normally receive an email from PayPal displaying our order. For some reason the whole Custom Order page has disappeared. We are running a static website so we're not sure what could have gone wrong. The way the Cart worked was by having the user type in a word or name in a field before hitting submit. The next screen would display that word or name in various photographs. The user would then be able to click on each individual letter to pick a different letter/photo from a list. They would then be taken back to the main Customize Your Order page to view their name or word. They can then click submit to be taken to PayPal to complete their order and an email would be sent back to us from PayPal so we can ship their correct photos from the shop. We need this recreated and have the photos renamed to match our system.
