online questionnaire (presentation and saving responses in database)

I have a small online application designed to create and deliver 5 types of questions to the users. I need an experienced programmer who has good designing sense as well to develop a small functionallity to present the questions to the user one by one and save the user responses in the database after evaluating them. Someone who has previously worked on similar projects will be preferred. Please find attached the flow chart of question presentation and save. Details of the flow chart (to the level of db field operations and all logic) will be provided to the shortlisted programmers. Please note that this functionality will be developed for an existing system which has all the questions stored in the database. You need not to create any db tables or any authentication process. You just need to code the documented process to achieve the desired result. Coding must be done as per standards (codes must be commented properly so that anybody knowing php can understand what the code is doing). This is a very urgent project, so a prompt response will be highly appreciated.
