Write a script to read/write MySQL records

I need a script to fetch all records in a MySQL table (maximum of 20). The value for each of the records must be represented by slider bar on the web app (value will be a number from 0 to 100). The user will change the value via the slider bar. When the slider bar is moved/adjusted in the web app, the web app must update the MySQL database value for that record which has changed. I have attached a mobile template which I suggest you embed the script. Slider bars do not have to be very fancy aesthetically. I am looking for functionality on iPhone/smartphone most of all. The records in the table have the columns (string_id,string_name,status). 'string_id' is the unique key. The human readable name is 'string_name'. The slider value is 'status'. App should look roughly like this: Zone 1 <----string_name --------------------o------------- 66 <----status value represented on slider and as integer Zone 2 -----o----------------- 20 (another example) See attached mobile template.
