116876 Advanced Form With IP Limiting

I need a form, in any codeing possible. The easiest would probably be php, cgi or with cookies. Heres what it needs to do: -Limit registration to one per ip address -Limit registration to one per email -mail form to suggested email address -several fields will be required It will need a "thank you"/"success" page and a page for users that get denied because they have already registered. Further details will be provided (fields needed, etc.)
