An application that concentrates sent tests

I need make a program mobile friendly (responsivity) that I can list all results of submitions done by external public. I need also to see the graph and export this graph in a PDF file. Also, it needs to have a filter list by date, name of person (partial or complete name) and by sex (male or female). This work must be done using a dictionary because I need it in portuguese and English. If I want later to translate to another language, I will just create another TXT with all labels. Another feature: in the list must be posible select two or more results and build a chart with the all done tests. Supose the same person did this test o month ago and made again yesterday. I need comparate the test results by seeing different color lines. I need a very nice design for the screens and all screens must be accepted before you start. In resume: - App in PHP; - Responsive to mobile; - Dictionary with labels and questions (the system must read the dictionary TXT files available - ex.: English, portuguese - and load all of them automaticaly); - External formulary whith 200 questions as on [login to view URL] (please do not submit tests on this link); - This app needs to process the graph by using the algorithm available; - List all submissions; - Actions: see the basic filled fields (as name, email, phone, etc), see the generated graph, export PDF file to print or save; - Filter by partial or complete name, by sex (male or female), by send date, by city, by state. - City and state (province) must be taken from postcard service or another available; - Dump database backup in a just one click; Areas: - Admin area: listing and searching all tests sent. - Email: send the email with the graph in PDF attached. - Public area: Test form ( you can split all questions in 25 to 25 questions per page showing the remaining pages or percentual).
