Drupal - Card On File not working for Recurring Framework

We have implemented Recurring Framework module and Card On File module to process recurring payments. However, only the first order is getting created and processed. All recurring order payments are failing to go through (Payment failed - hard decline). I suspect this is because the expected option to choose "save card information" is not appearing on the checkout form. I need someone to look at this to determine why the Card On File functionality is not working with Recurring Framework to properly process recurring payments.
