I am launching a website and need to create a form that is similar in complexity to the following examples: [login to view URL] [login to view URL] Aspects to consider completing the form are: 1. There are seven tabs in the form with ~+30 questions in total 2. Tooltips will be required 3. A progress bar at the top is required 4. Custom font, color scheme and CSS is required 5. A payment gateway will be required at the end of the process, the amount charged may vary depending on the product ordered 6. The ability for Users to save the form would be preferable 7. When entering in address information if there is an auto fill feature for Australian address this would be preferable. 8. A custom object where content displayed if a string of text is click on is desirable (e.g. > Click for further information on Step 1) Note, we will provide all the content for the form. We are experienced developers and are building the form using PHP on Spark Laravel platform. Please review the examples and then come back to me with a preliminary quote on how many hours you think the total job would take as well as your hourly rate.  Come back with thoughts on approach to the job, questions, expected time, indicative quote, and relevant work experience.
