170783 Joomla form

I need a form on our Joomla website where customers can answer a couple of questions and upload files. The result should be either emailed to admin or be visible in backend. The form has to be able to handle checkboxes and input fileds. The purpose is to have customers who wants to sell their house/apartment enter their information on a webpage and upload pictures to us. This is a Joomla installation and there is a component already doing this calle facsimile forms or similar that can be used. I need help creating the forms Website name and fields in PM. Payment thru escrow only.
