1 Introduction:
Our business helps clients deal with work and information overload. For the past 5 years we have been using an MS Excel survey to test the impact of our workshops. We call this The Overload Gauge. I am considering converting this into a SIMPLE WEB based tool. While I have made every effort to be simple and clear, I am open to ideas or advice that could make this simple to use and maintain
1.1 Functional Objectives:
1. Develop a Web based data capture tool to store participants Overload Gauge data BEFORE and AFTER our Overload Workshop.
2. Provide appropriate reporting either by programmed reports by a report generator tool.
3. E-mail appropriate reports to workshop participants and workshop sponsor. .
1.2 Functional Requirements
Three basic functions are required in this system:
1. Administrator function: Including Tables Setup: Add, Edit/ delete & Save functions.
a. Survey Setup: One table for the whole system. Includes 20 survey statements and related totals in 3 groups.
b. Project Setup: One for each project to includes organization (Client) and sponsor data
c. Participants Table Setup: One for each project. Includes participant’s data as well as before and after survey data.
2. Participants’ input, web programs: two almost identical web based programs:
A. The FIRST Input: This updates each participant’s database.
1. Access by Client project code. Display Project welcome screen.
2. Access participants record by combination e-mail and unique participant code.
3. Prevents re-entry of data. Provide error message or E-mail administrator for support
4. Capture participants’ FIRST response to survey statements.

B. The SECOND Input: (Very similar to the first input)
1. Access by Client project code. Display Project welcome screen.

3. Maintenance and Reports
• Maintain, manipulate and correct individual all fields in all tables in the data base.
• Reporting Functions, provide reports ( Or provide report writer/ generator to create provided examples)
PLEASE Note: THIS IS THE MOST IMPORTANT FUNCTION OF THE SYSTEM.
• Be VERY clear of your plan for easy delivery of this function before you start.
• I am open to possibility of using an administrator initiated report writer provided that report 4, 5, 8, & 9 can be e-mailed in good presentation quality in exactly the table format illustrate on the following pages along with come accompanying text.
