Web Design

This is my third time listing this project, only SERIOUS bidders please. I need a professional, elegantly designed website for my Dessert Cafe. One template is ok for the whole site, but it must be a custom designed template NO PREMADE TEMPLATES! The site will require a total of 9 simple pages. The home page will require a form to sign up for my mailing list. One page will be a customer survey which will allow me to track results, and direct the user to a thank you page linking to a printable coupon. The rest will be simply information pages. You must be able set up the mailing list & survey for me. I will provide all content for the website upon acceptance of a bid. No pre designed templates please, only custom work. I have attached a copy of my logo to this project description. This website is for a small pastry shop and cafe. Thanks for bidding. Mock-ups are appreciated. Portfolio's required. I will need this project done quickly.
