I need to have survey data captured into Excel (template supplied) off 491 single page, scanned surveys. There are 44 fields per survey and the majority of them (34) are simply checkboxes (1 or 0).

Hi Sir/Mam, I shall get all the 491 surveys completed with in 2 days. And the amount shall be 200 AUD for all 491 surveys. I will make sure delivery will be done with accuracy and quality. I have team of 10 members so delivery will done asap along with quality since all of them have got experience in BPO. I will incorporate three stage of process in transferring and qc 1) Data transfer from pdf/scanned images to excel.-Manual or semi automated(whichever is accurate). 2) Qc by agent 1. 3) QC by Me.
