data entry - transfer list to ms word template

Multiple lists of data (names and addresses) converted into Microsoft Word template. Template is provided. Accuracy is important (will be printed and used in mailings). There are 900 name and address combinations, all are unique and there are no duplicates. Addresses will be coming from scanned documents similar to outline of yellow pages. Example below. Data will be in lines: Samsonite, Susan - 200 east fourth street, kettering, ohio 45459 Will need to be reorganized and transfered to match this outline: Susan Samsonite 200 East Fourth Street Kettering, OH 45459 There is no room for error. Order of the name, correct spelling of the addresses, zip, city and state are necessary for completion of this project. If interested, will provide additional work of same capacity (with new lists of data). I am accepting bids and will select the one I feel is most appropriate.
