Contact Management website - Step 1

Looking for a fully functional website built as an example of where to go from here. This is a contact management site which should hold two main tables to start; Contacts and Companies. For Companies, one field - CompanyName - will suffice for now. The Contacts table should have fields for EntryDate (date when record added) FamilyName, GivenName, multiple ContactNumber (which can be assigned a value such as mobile, home, work, etc.), multiple Email (which can be assigned as work, personal, etc.), Salary, SalaryDate (date when salary updated), English (which can be assigned the values of none, below, average, well, fluent, native), and Birthdate. Birthdate should produce a caculated current age on the UI. ******Except for CompanyNames and date fields, 1) I want to be able to edit the fields on the fly without page refreshes!! 2) Navigation (simple search) - typing a last name or partial last name should produce a list of matching contacts - FamilyName, GivenName, CompanyName. Clicking on one of the matching contacts will update all of the fields listed above, including CompanyName, to reveal the entire record; also hopefully this can be done without a page refresh. I want it to act like an application you would find on your computer, where only the field data is refreshed, rather than a website with constant page refreshes. I would like to maintain a MySQL backend. A combination of PHP/AJAX for the front-end sounds more likely, but I am open to front-end technology and/or framework suggestions. I have a one page 'under construction' site and will provide CPanel admin access for you to develop/load your project. Security for the scope of this initial project is NOT an issue. You can make up the data. Eventually, this will be a fully functional contact management website (web app) addressing security, speed/ease of use, data integrity, user login, privileges, document storage with linking, while holding the information for thousands of individual records. I'm hoping to continue on with the same developer to further develop the entire website, which will have many future ongoing customizations - think sales pipeline, email text integration, LinkedIn merging, phone integration, mass mailing. My legacy Contacts table alone has over 50 fields with over 10 tables in the db.
