I need to have a page that has a working contact form identical to: [login to view URL] It would suffice to simply copy this one page and keep all of links linking back to the original site. I only need the contact form to go to a custom email. The site should look identical and the form with the attachments, etc should all work to input to email. Upon completion, I would only need the files so I can upload to my server. After testing and everything works, the job will be complete
