Service Form

This app converts the service report form to mobile app. Please see the attached screens. The flow is as follows, 01 Basic Info: enter the data and click next to go to next screen. Customer Name, Equipment is mandatory. 02 Problem Description: enter the data and click next to go to next screen. Nature of problem is mandatory 03 Observation and reading: Depending upon the selection of the drop down of "Input Voltage" the caption of the three text fields below should change. 04 Observation and reading - battery: A button "+" to add the text field below every battery "B1, B2" 05 Final Remarks: Capture signature online and his email Other functionality required: 1. Store the data locally 2. Send two email from the mobile email; one to customers email ID and second to the company mail ID 3. Export the data in excel format
