Format Document Text, Links and Images into PDF Ebook

I am looking for someone experienced with taking text that has already been written and edited, and making a nicely formatted ebook from the text. I currently have a .docx that is 88 pages long, about 44 regular size document pages, of edited text, images and links that needs to be made into an ebook. The ebook would need to have: -Image front and back cover (I would provide the images for the covers) -The side bar needs to show chapter headings -The pages in the table of contents will need to be changed to match the pages the chapters show up on in the pdf. -The images will need to be inserted in the correct areas, and all the links will need to be clickeable, opening up in new pages on the internet. -Light formatting (making sure bullets are lined up, all lines are even, etc.) This project has an absolute deadline of December 20, 2010. If possible, I would like to see samples of previous ebooks created. Thank you.
