307905 jquery form

I have a database db1 and table tbl1 with 4 fields: id, name, birthday, country I want to have the following form that defaulted to display name, birthday, country where ID = 0 form element: select name: <select drop down> id: <hidden input text> name: <input text> birthday: <input text> country: <input text> save button and jquery in the "select name" drop down that list of all the name in database and 'add new student' as the last option in the drop down . if one of the name is selected, the form will display the name corresponding to the select drop down and user can modify the data and click on save button to save the data to tbl1 If add new student is selected, the form will be empty and user can fill in the form and click on save to save the data to tbl1 this should be easy and quick... the first one who can give me demo with reasonable bid will obviously get the project
