89371 Simple Petition PHP Script

Hello, I need a simple script for a petition site, with a small control panel. I do not need any graphic support,just pure, well written php. All of the information will be stored in a mysql database. The Script will have three pages. Main Page of Site -There will be a box listing a form letter that can be edited via the control panel. THe user will be asked to input their information to sign the petition. At the bottom of the page there will be listed all ofthe individuals who have already signed the petition. There will then be 6 form values: First Name: Last Name: AU ID Number: (This is to be encrypted when it is stored, and entered like a password with *s. I can view it only through the control panel). This field will be restricted to seven numerical digits. Class: (drop down menu: Choices include 2010, 2009, 2008, 2007, and Alumni. If Alumni is selected the AU ID field is removed and not needed). [-at-] [login to view URL] Email Address: This value will make sure the email address entered is [-at-] [login to view URL] Timestamp: This will be a hidden value, but the mysql database will record the time someone signed the petition. When the user fills out the form and hits a button saying "I Wish To Sign my Name to this Petition", the first thing the script will do will be to make sure no other user with the same AU ID # variable signed up for the petition. If the same user tries to sign up, the user will be told they have already signed the petition and cannot do so again. If the user is indeed a new user, their information will be stored in the database. Additionally, as they hit the button, emails containing the letter will be sent off to a select list of email addresses I give you (and can be edited in the control panel). The Email address will be signed with the following variables in brackets: [First Name] [Last Name] Class of [class] [email] AU ID Number Upon Request The final thing this page does when the user signs up for the petition, is it sends the user to a page thanking them for signing the petition. Listing Page: On this page a table will be shown listing all of the individuals that have signed the petition. At the top, the total individuals who have signed the petition will be shown. The table values will go across like this: Petitioner #(1,2,3..etc) First Name, First Letter of Last Name, and Class. Control Panel: This control panel will be password protected and only accessable by myself. It will allow me to edit the form letter that the site will be using, as well as the email addresses that the letter is being sent to by each petitioning user. Through the control panel, I'll also be able to see a table with all of the variables uncrypted,including AU ID #, of all of the people who have signed the petition.
